磁共振成像在循环系统疾病的应用磁共振成像用于心血管病检查时，扫描序列用自旋回波技术(SE)，回波时间(TE)保持28.56毫秒,脉冲重复时间(TR)据心率确定，即TR＝R－R时间。为缩短扫描时间可采用多层面扫描序列，这样只略增加TR时间即能一次获得连续4～16个层面,每层有4～8帧不同的T1和T2加权图像。运用较小的翻转角度，如10°～15°，也能缩短TR和扫描时间，可在数秒内获得图像。心脏三维磁共振重建术与心电图门控联用,可用于电影磁共振技术,可获得四维图像，对研究心脏具有实际意义。在后天性心血管病方面，对下列疾病有诊断价值。①缺血性心脏病。在慢性心肌梗死，梗死区被纤维瘢痕所代替，回波信号减弱，T2缩短。急性心肌梗死磁共振信号强，T2延长，并发现局部性心肌壁变薄。在心肌梗塞部磁共振可以显示残余的正常心肌，可帮助确定能否做冠状动脉搭桥手术。②心肌病。磁共振可以准确地判断肥厚性心肌病的有无、病变范围和程度；对充血性心肌病可显示心室扩大程度。可以发现肥厚性心肌病的某些变异型。③心包疾病。正常心包在磁共振上为低信号，厚度为1.6±0.4mm（范围为0.8～2.6mm）。亚急性心包膜增厚在自旋－回波成像中呈中等强度信号，慢性缩窄性心包炎中，心包膜纤维增生伴有钙化，信号弱。磁共振除能观察少量心包积液外，还能区分血性还是其他成分的液体。④胸主动脉疾病。心电图门控磁共振可以完全取代B型超声波和CT检查,不用对比剂即可清楚地显示主动脉的解剖结构、病变大小和范围、有无血栓、管腔扩张或狭窄以及邻近血管的关系。在先天性心脏病的诊断方面采用心电图门控磁共振对先心病解剖畸形诊断准确率达80％以上，而且能够对左向右分流的先天性心脏病提供生理性信息。磁共振是一项无损伤性检查方法，对诊断心脏大血管疾病具有如下优点：借助于血流（无信号）代替对比剂，观察其解剖结构，联用心电图门控或手动门控磁共振可提高心脏腔门结构的清晰度，判断心功能情况；可任意做横断面、冠状面、矢状面等，在每个断面内无死角。目前，磁共振存在的问题是扫描时间长，瓣膜病变（关闭不全）仍需主动脉造影或左室造影，分辨率有待进一步提高。
